Health Requirements for Registration
The deadlines for these requirements are as follows:
2024-25 School Year
- Dental: Upon enrollment, or May 13, 2025
- Physical: Upon enrollment, or October 15, 2024
- Vision: Upon enrollment, or October 15, 2024

Physical Exams
A “sports physical” does not meet the requirement. A current physical is one that was completed after August 18, 2022. All students must be up-to-date with the State of Illinois immunization requirements. Physical examination forms are available above and on the Health Services page.
Sports Physicals
“Sports physicals" are required for all students participating in athletics. Sports physicals are good for 395 days from the date of the physical. A State of Illinois physical can be used as a sports physical.
Medication Policy
The policy of River Trails School District 26, in accordance with Recommended Guidelines for Medication Administration in Schools (Illinois Department of Human Services and the Illinois State Board of Education) is as follows:
- Administration of medication to a student while at school is discouraged unless it is absolutely necessary for the student's academic participation.
- Medication (including all over-the-counter medication) to be administered at school must be prescribed by a licensed prescriber (i.e. physician, dentist, podiatrist).
- All medication must be in the original container and have an appropriate label.
- Medication will be administered under the supervision of the registered nurse.
- Long-term medication requires annual renewal orders.
A Medication/Treatment Order and Parental Request must accompany the medication at school.
